Why This Course Matters
Care plans are one of the most important tools support workers use when providing day-to-day support.
A care plan tells you what support a participant needs, how that support should be provided, what risks you need to be aware of, and what is important to the participant. Following the care plan helps ensure support is safe, consistent and person-centred.
Understanding how to correctly read and use a care plan also helps you recognise changes, respond to concerns, work within your scope of practice and know when something needs to be reported or escalated.
